As the cost of healthcare continues to rise, many states are struggling to maintain access to essential medications for individuals living with HIV. A recent report highlights the impact of federal funding constraints on state AIDS Drug Assistance Programs (ADAPs), which provide critical support to those in need.

State-Level Cuts and Considerations

At least 18 states have implemented cost-containment measures, including lowering income eligibility thresholds, restricting medication coverage, and tightening enrollment rules. For example:

  • Florida has reduced ADAP eligibility from 400% to 130% of the federal poverty level, affecting over 12,000 clients.
  • Pennsylvania has lowered its eligibility threshold from 500% to 350% of the federal poverty level, impacting approximately 1,600 clients.
  • Kansas has dropped premium-assistance eligibility from 400% to 250%, while Delaware and Rhode Island have reduced their eligibility caps.

Federal Funding Pressures and Constrained Budgets

The report notes that flat federal funding for the Ryan White HIV/AIDS Program is a key driver behind state-level cost-containment measures. Despite rising enrollment and medication costs, federal appropriations have not kept pace, leaving states to shoulder more of the financial burden.

A sustained investment in HIV prevention, treatment, and support services is essential to maintaining access to treatment and ultimately ending the HIV epidemic in the United States.
